Roshan Ravindran picks me up in his black Rolls-Royce. With its orange leather seats, twinkling star lights on the ceiling, and cushions embroidered with the logo of his aesthetic clinic, the limo is worth well over £300,000, he says.

He laughs when I reach for the handle. ‘Don’t worry, the door opens itself.’

It’s fair to say that business as a cosmetic aesthetician in Manchester is booming.

Indeed, Ravindran – known as Dr Rosh – knows the secrets behinds some of the most famous faces in the UK. Whether it’s an A-list footballer, Love Island star or a high-end influencer, they confess their insecurities in the comfort of his Manchester clinic, Klnik, and then trust him implicitly to fix them.

‘I see everyone,’ he tells me. ‘You’d be shocked at some of the names.’

His client list ranges from influencer mega-star Molly-Mae Hague26, to model Lottie Moss27.

Love Island contestant Ekin-Su Cülcüloğlu, 31, dropped by Dr Rosh’s clinic before entering the Love Island villa for the second time in 2024 to have some ‘volume added to her cheeks’. He also injected ‘tear trough filler to get rid of under eye hollows’ and gave her ‘definition to her jaw’.

Right now, Dr Rosh’s most popular procedures are lip flips (a Botox adjustment to roll the top lip upward just enough to make it look fuller, but still natural); softening of smile lines; something called ‘fox eye lifts’ (using ultra-fine threads to gently pull the eyelid and brow outwards and upwards) and aforementioned tear trough (under eye) filler to get rid of bags. These one off individual procedures start from £245, however combinations can rack up the price fast.

Manchester influencer Madison Sarah, 28, is a regular at Dr Rosh’s clinic, for example, and has had ‘very small’ amounts of Botox in her forehead, temples, and laughter lines from her nose to her mouth – all with the aim of softening fine lines and preventing wrinkles. Her mum is also a client to ‘reverse her ageing’. She had a lip flip, and Botox to relax forehead wrinkles.

Dr Rosh, 44, is a particular favourite among Love Island contestants, with Harrison Solomon, 23, from this year’s series, recently dropping by for tear trough filler. An alumnus of Love Island 2024, Grace Jackson, 27, has also seen Dr Rosh for a fox eye lift, tear trough filler and masseter (jaw muscle) and neck Botox. The cost of any one of these treatment combinations could easily soar north of £2,000.

Of course the early twenties is very young indeed for aesthetic surgery. Originally, when he opened his Manchester clinic in 2017, Dr Rosh only treated clients aged 25 and up, but with so many bad fillers being done on younger people, he began taking on 18-year-olds – who mainly wanted lip enhancements – to correct work before it worsened.

Dr Rosh offers a tantalising glimpse into the entitled nature of many other celebs, whom he won’t name. ‘I could show you ten messages that I got just this morning from celebrities and influencers asking me to inject them for free in return for a social media post,’ he says. ‘I just don’t respond.

‘I have even had people ask me to pay them to come and see me. You wouldn’t believe some of the requests we get.

‘No one gets injected for free. My most famous clients pay full price. If I post the work (on social media), we come to an agreement and they pay the cost, but everyone pays.’

He has also turned down celebrities, even when they’ve offered him large sums.

‘I’ve been offered loads of money, or a slot on a TV show, I don’t care. I don’t want to be associated with an unnatural face. I wouldn’t sleep at night. I could make a huge amount of money if I was willing to make people’s lips too big.’

These days Dr Rosh doesn’t need the publicity, since – with 600,000 Instagram followers and than four million likes on TikTok – the man is almost as famous as some of his clients.

His Wilmslow clinic is sleek and glass-fronted, tucked in a private, modern business park between a high-end kitchen showroom and Fine & Country estate agents. Inside, I’m handed a £5 bottle of Fiji water and led to his office. There are floor-to-ceiling mirrors, a leather desk, and a sofa that looks like it’s never been sat on. The office door can only be opened from a hidden button next to his chair.

‘I am a psychologist as well as an aesthetician,’ he begins.

‘My job is to give people confidence and make them feel happier. I want to improve their psychology.’

His clinic has a four-week waitlist and a team of 15, including – extraordinarily – three full-time staff members to manage social media.

Rosh has now expanded to clinics above Harvey Nichols in Manchester and Harley Street in London.

His influence, he insists, has changed the face of Cheshire, quite literally.

‘I started by doing a lot of corrections and that got me my name. In this area, there were a lot of beautiful people who got botched. My goal was to keep them beautiful, not overfilled.

‘Not long ago in Cheshire, you could walk into one of the bars and see all of the cheap fillers. I have corrected so many people now.

‘My word-of-mouth coverage is very strong. And I ruthlessly stand for not overfilling.

‘My income from one person is minimal – I maybe only see them a couple of times a year, but because I do such a good job – I get five new clients from their word of mouth.’

Business is clearly thriving. Next week he and his family are flying to Prague to stay in the £2,500-a-night Mandarin Oriental’s presidential suite for work extended into a holiday, before spending Christmas in Asia.

He is, unsurprisingly, being courted by TV producers.

‘At one point, I will have a Netflix series. We have about four offers on the table right now from top television companies. It’s important that I choose the right one.

‘I would want it to be reality television, honestly the things that go on in this clinic, they’d be spoilt for choice.’

International expansion is on the cards too. ‘I will open a clinic in Dubai next. I won’t be there permanently, but there is a huge market there for correction.

‘We are currently in talks with one of the top hotels in the world there.

‘I have had offers in Birmingham, Newcastle, Scotland too.’

His clients now fly in from Australia, Canada and Dubai weekly, he says.

Success has not fallen into his lap, however. Dr Rosh almost dropped out of medical school because he struggled with the academic side.

‘I have always been so much better with my hands,’ he said.

‘After third year, when it became practical, I became the best in my class.’

Following his parents’ expectations, he went into cardiology and worked as a Junior Doctor at Wythenshawe Hospital before moving to the Cardiac MRI Research department at the Texas Heart Institute in the US.

He eventually began GP training in the UK, where a colleague introduced him to aesthetics.

His first Botox training course was in 2011, and it was at this point, he says, he discovered he had a real talent.

Soon, major aesthetic and pharmaceutical companies began to take notice.

They flew him around the world to train other doctors in aesthetics. He studied under leading practitioners and refined his technique.

‘By the end of two years, I’d become something different. I’d absorbed all of their knowledge.’

He chose Wilmslow to open his clinic in 2017, largely because it was right on the doorstep of ‘WAG City’.

‘Aesthetics is cutthroat. When I opened here, people saw me as competition immediately. When I made a pound, it was like I’d stolen it from someone else.’

He tells me staff were poached. Competitors interfered in his personal life. ‘Wolves were circling. I was naïve.

‘In the UK, anyone can do aesthetics. Someone can do a five-day course and start injecting. That’s terrifying. You can cause blindness, vascular occlusion, permanent damage. I see it every week – I’m the one correcting it.

‘You wouldn’t be allowed to inject medication unsupervised after just a month of medical school. Yet people are injecting filler after a weekend course. It’s madness.’

He shakes his head, clearly passionate about the topic.

‘I know I’ve changed the industry here. And I’m nowhere near finished.’
